A unique management plan designed specifically for you
Some people benefit from a structured care plan. This can take various forms. You may have a condition that requires medication and regular visits to a GP or other care professional. If we feel your treatment will benefit from us communicating with your GP, we will ask your permission to do this.
A good relationship with a team of health professionals, all dedicated to your care, is good for your long term health. We can also discuss and help educate GPs about the role of complementary health, particularly when it comes to chiropractic.
How does a chronic disease management plan work?
You don’t have to worry about preparing anything in advance. In our first consultation with you, we’ll discuss your medical history and from that, we’ll glean whether a care plan will be of benefit to you.
You may be eligible for what’s called a Chronic Disease Management Plan (formerly known as Enhanced Primary Care Plan). This scheme is for people who have a condition that requires care from multiple practitioners. Conditions are usually chronic and present for six months or longer. It is coordinated by your GP and is part of the Medicare Benefits Schedule (MBS).
